REGISTRATION INFORMATION

Welcome to Plain Local Schools

Plain Local Schools has moved to an online registration process for K-12 student registrations for the 2025-2026 school year using FinalForms. This streamlined process allows families to complete required paperwork online, making registration more convenient and efficient. Please note: an in-person meeting may still be required to verify and approve required documents. We look forward to helping you join the Golden Eagles family!

Registration Process 

  • Visit FinalForms: Go to https://plain-oh.finalforms.com
  • Create Your Account:
    • New families: Under Parent, click New Account and follow the setup steps.
    • Returning families: Log in under Parent with your email and password, then click Add Student in the upper right corner.
  • Complete Enrollment Forms: Fill out all required student information, including emergency contacts, health forms, and any needed permissions.
  • Upload Required Documents: See the checklist below for what you need to submit.
  • Review and Sign: Carefully review all forms, digitally sign, and submit.
  • Watch for Confirmation: You’ll receive an email confirmation from Final Forms once student account is created. 
  • Follow Up Meeting: You will be contacted by a building secretary to schedule a document review appointment and schedule a tour of the school. 

Document Checklist for Registration

Please have these documents ready to upload to FinalForms or bring to your in-person appointment:

  • Student’s birth certificate
  • Immunization records
  • Proof of residency: Example: lease agreement, mortgage statement or current utility bill. Please note a utility bill must be gas or electric; cable/cell phone bills will not be accepted as proof of residency.
  • Parent/guardian state issued photo ID
  • Custody documents (if applicable)
  • IEP or 504 Plan (if applicable)

Instructions & Best Practices for Uploading Documents in FinalForms

Gather your documents ahead of time
Before you begin registration, be sure to have clear copies or photos of the required documents ready. Commonly required documents include:

  • Student’s birth certificate
  • Immunization records
  • Proof of residency: Example: lease agreement, mortgage statement or current utility bill. Please note a utility bill must be gas or electric; cable/cell phone bills will not be accepted as proof of residency.
  • Parent/guardian state issued photo ID
  • Custody documents (if applicable)
  • IEP or 504 Plan (if applicable)

Check your file format and size
FinalForms accepts most common file types, including PDF, JPG, PNG, and DOC.

  • Make sure each file is under 5MB to avoid upload issues.

  • If using a phone to take pictures, check that they are clear and not blurry.

Upload documents in the correct section
When you reach the document upload section in FinalForms:

  • Click Upload next to the correct document type.

  • Select the file from your computer or mobile device.

  • Wait for the upload confirmation (you may see a checkmark or green bar indicating success).

 Double-check before submitting

  • Confirm that each document uploaded correctly and is attached to the right section.

  • If you are unsure, review each section or ask for help before submitting.

Ask for help if needed
If you run into problems uploading documents:

Important Reminder: You can also bring hard copies to the school if you’re unable to upload them online. 

Even if you upload your documents in FinalForms, the district may still require you to bring the original documents in person. This is typically requested if the uploaded copies are unclear, incomplete, or cannot be verified for legal or residency purposes.

Please keep all original documents handy and be prepared to provide them at your school building or at the District Office if requested. 

Tips for Taking, Scanning, or Downloading Important Documents

We want to help make sure your document uploads go smoothly! Here’s how to prepare clear, usable files for FinalForms.

Taking Photos of Documents (with a phone or tablet)

  • Place the document on a flat, solid surface (like a table).
  • Make sure the area is well-lit but avoid glare or shadows.
  • Hold your phone directly above the document, not at an angle.
  • Capture the entire document in the frame (no cut-off corners or edges).
  • Check the image before uploading — it should be sharp, readable, and not blurry.

Tip: If your phone has a “document scan” feature in the camera or Notes app, use it! It will automatically straighten and enhance your image.

Scanning Documents (with a home scanner or app)

  • Set your scanner to PDF or JPG format at 200–300 dpi for clear resolution.
  • Choose black-and-white or color, depending on the document.
  • Use a free scanning app (like Adobe Scan, Microsoft Lens, or Genius Scan) if you don’t have a home scanner.
  • Review each scan before saving make sure all text is easy to read.

Downloading Electronic Copies (if available)

  • If a document is already available online (such as vaccination records from your doctor’s portal), download it as a PDF.
  • Save it with a clear file name, like “JaneDoe_Immunizations.pdf.”
  • Double-check that the file is complete before uploading.

General Best Practices

  • Keep file sizes under 5MB per document.
  • Use clear file names to stay organized.
  • Keep a backup copy for your own records (either saved on your computer or printed).
  • If you’re unsure or need help, call the school office or the Administration office at 330-492-3500
